Job Description

Main purpose of the job The Infectious diseases Focal Point main responsibility is to coordinate and facilitate the screen-ing, treatment and follow up of patients with infectious diseases, in particular HIV, Hepatitis C, Hepatitis B and Tuberculosis. She /He is in charge of organizing the multidisciplinary management of infectious diseases and linking the different departments internally and externally. Responsibilities and tasks • Develop and implement an individualized comprehensive case management plan for each patient diagnosed with infectious diseases, in particular HIV, Hep C, Hep B, TB. • Ensure that infectious diseases patients receive multidisciplinary consultations and diagnostic investigations according to MSF protocols and organize the follow up appointments with the multidisciplinary team (doctor, psychologist, psychiatrist, social worker and CHW) • Provide the patients and caregivers with information about the management plan of the illness • Work in collaboration with PSAM for patients follow up and work organization by CHWs and social workers in charge of following IDs patients • Ensure phone calls and in person communication with camps, Welfare organization, triangular centres, etc. for follow ups of the patients and to ensure insurance booklet for patients when possible • Provide administration department with the list of Hepatitis C patients under insurance coverage on a monthly basis in collaboration with Social Worker • Notify Nurse Team Supervisor in due time for cases/situations that require immediate attention such as loss to follow up, severe side effects and other complications. • Collaborate with referral nurse for referral or follow up or other movements • Contribute to quality of care in the clinic • Prepare, in collaboration with consultants, case presentations for the regular meetings with ID specialist and ensure that MSF protocols are respected. • Organizing and participating in regular HCV meetings, in particular for multidisciplinary case management of Hep C patients • Organizing issuing of Hep C card • Ensure proper data collection, compile database and submit monthly report to Nurse team Supervisor. • Ensure confidentiality of the patient is respected
