Job Description

Main purpose of the job: The Community health worker will provide information at community level, and will strengthen the link between the community and MSF health structures. Responsibilities and tasks: • Collect and report specific information related to target population: diseases, food security, and general status of patient, constraints and strengths, vulnerability, level of information, living conditions and other risks related to the marginalized situation of the population. • Deliver information to his/her community on specific health topics and about MSF, and the services it provides • Consolidate the link between his/her community and MSF services; give advice on how to better involve his/her community in the activities of awareness and prevention • Monitor the field dynamics of our target populations (new drugs on the market, increase of numbers of drug users, etc.) and actively contributes in the preparation of an updated plan of the activities the activities in case of significant changes. • Participate in active follow up of the patients and support the defaulter tracing activities (by phone, house visits and/or outreach activity) • Be responsible to receive the feedback and perception of MSF services from the patients. • Accompany patients to hospitals/clinics/laboratories during referral. Manage administrative process and make sure that the patient receives care. • Inside the clinic, guide the patient and facilitate the access to different services • Share any relevant information about the patient that could help consultants to provide better follow up
